Patients can register with the practice by bringing their medical card or completing an HS22X form. The patient registers with the Practice and not with an individual GP.
Each new patient will be offered a health check at their first consultation. This will provide a general check-over, with blood pressure, weight and risk-factor analysis. It also allows computerization of records.
Patients must phone the surgery to be given a time to attend. It will not be possible to book this more than 48 hours in advance. There is a system to deal with emergencies.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
